- If you're unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.
Discount applies to Physician Services only. Discount does not apply to X-Ray, Ultrasounds, Lab Tests, Medication or Supplies. Self-pay patients are required to pay when services are received. We do not bill self-pay patients.

For patients treated for injures from an automobile or work injury, we will attempt to receive authorization from the appropriate carriers before your visit. If prior authorization is not received, you will be responsible for the visit.
It is very important that your doctor have your up-to-date address and phone number, so we are able to contact you. Please make sure the receptionist is aware of any changes. Also, please make sure we have both work and home phone numbers and another emergency contact number if possible.
You will be responsible for the bill if the service is not a covered benefit; the service is not deemed medically necessary by your insurance company; or your insurance plan requires you to pay deductibles, co-payments and/or co-insurance. For all patients who must personally pay all or part of their healthcare bills, we accept check, MC, Visa, Discover, American Express or cash, Upon receipt of our billing notice showing your balance due, you are expected to make payment in full or contact us to make other arrangements. Our billing office staff will be glad to discuss your insurance with you or your designated responsible party. You are encouraged to ask us any financial questions relating to the services you received. Our staff will be eager to assist you.
Please contact your pharmacy and they will fax to us your request for a refill. Please allow up to 24 hours to process.
